Admitting

Listed below is the ACE service admission criteria.

ACE Service Admission Criteria
  1. Age over 70
  2. Dementia or history of delirium
  3. Comorbidities and factors known to increase risk of hospital complications including a history of recent falls, mental status changes, skin breakdown, urinary incontinence, and prior recent admission
  4. Residents of assisted living or skilled nursing facilities
  5. Patients who are followed or who have been seen in consultation by the staff of the geriatric division
  6. Patients who have been evaluated in the Geriatric Evaluation Clinic
  7. Specific referrals to the geriatric service
Admitting Process
  1. Admitting Structure: The team consists of two interns and one resident. The team is on call every other day, each intern is on call every fourth day. Interns and residents no longer take overnight call during the week. The intern on call and the team resident will see admission on a call day until 7 pm. A night resident who sees admissions and performs cross cover duties is on from 7 pm until 7 am. The team can take up to 2 "on call floats" on the day of admission, and then can take up to 5 new admissions until the time cap of 11 pm. On the off call day, the team can take one "short call" admission that is an appropriate patient for the geriatric service.
  2. Admissions from the Emergency Room: The ED will contact the resident directly with an admission. Admissions for the day for each team can be viewed under the Group Patient List function in WedCIS.
  3. Clinic admissions: The attending involved in the admission should contact bed control/admissions (6-2041) and should call the resident and the attending on the team directly.
  4. Hospital to hospital transfers: Can occur if bed control confirms availability. This should be done by the attending on service.
  5. Direct admissions from skilled nursing facilities: Direct admissions can occur from facilities at which our faculty work, such as Carol Woods and Carolina Meadows, if a faculty member or clinician has evaluated a patient and determines them to be medically stable for direct admission to the floor. The faculty attending then must call the patient in to admitting and contact the team as above.
