summary information regarding diagnosis, screening, and management

Evidence-based Document: more complete information organized around "clinical questions", and presented as "recommendations", "rationale", and "resources".

Teaching card:

a downloadable pocket-size card which summarizes the information and is to be used as a reference tool when seeing patients

Audit form :

a checklist which transforms the screening, management, and prevention quidelines into quality of care markers. The audit sheet can be used to audit charts for quality performance. It can also be used by learners to assess their own care of patients, thus reinforcing instruction.
