Required Documentation
ALL students
- All students must document completion of required immunizations. Tetanus, diphtheria, measles, mumps, and rubella are required by state law and must be documented within 30 days of matriculation, or registration will be cancelled. The additional requirements for Health Affairs students must be completed prior to clinical rotations. Please note that the Hepatitis B vaccine series takes 7 months, and that you must have a titer 1-2 months after completion of the series. Immunization documentation must be submitted to Campus Health Services. See http://campushealth.unc.edu for online health history and immunization submission instructions. (UNC-CH graduates who attended UNC-CH within the past two years do not have to resubmit documentation. Any updates may be provided directly to Campus Health Services.) All students must request a printout of the CHS record and submit it to the Division of Clinical Laboratory Science.
- All students must document annually that they have read, understand, and agree to comply with program Policies and Procedures. All UNC and CLS policies apply whether the student is on-campus or off-campus. In addition, students are expected to adhere to the policies of the host facility. If the facility's policy (i.e., dress code) is more restrictive than the CLS policy, the student is governed by the facility policy. Acceptance of CLS policies and procedures is assumed with registration.
- All students must complete safety training annually. This will include on-site training on Orientation Day. Prior to orientation, each student must complete 5 safety modules on the Laboratory Training Library site (Electrical Safety, Fire Safety, Biosafety, Chemical Safety, and Ergonomic Safety). You will receive instructions on how to access this resource. Keep your password! You will be using it often. You must make >80% on each quiz. Print your quiz scores and bring to orientation. First-year CLS students
- All First-year CLS students must also document understanding of information found here.
- All first-year CLS students must complete the Campus Health Service Latex Questionnaire. Second-year students do not have to complete this form unless your status has changed.
Second-year CLS students
- All second-year CLS students must document major medical insurance coverage prior to beginning clinical rotations. If you need to purchase insurance, you are eligible for the UNC Student Medical Insurance Plan administered by Hill, Chesson, & Woody. (See "Insurance Information at http://campushealth.unc.edu.) **Continuing students interested in the accelerated program please note that the open enrollment period for this plan is July-September. You cannot be covered by this plan for summer rotations unless you purchase it in the first year, or unless you have a qualifying "status change".
- All second-year CLS students must complete Modules 1 & 2 of the UNC School of Medicine online HIPAA Training prior to beginning clinical rotations. Print your completion certificate and bring to orientation.
- All second-year CLS students will be required to have a criminal background check prior to beginning clinical rotations. More information (cost, etc.) will be provided before the check is performed.
Second-year CLS students completing a rotation at a Duke University Health System facility are required to complete training located at http://edservices.mc.duke.edu/ClinAffpdf.html, and submit documentation of completion. Students may also be required to fulfill additional requirements of other clinical sites (safety orientations, etc.). You will be informed of these requirements as we are made aware of them.
