UNC Physical Therapy Alumni Association

The UNC-Chapel Hill Physical Therapy Alumni Association was organized in 1969. The original bylaws governing the Association have undergone some modifications over the years, however, its main objectives have endured. There are three primary functions of the Association:

  • To encourage mutually beneficial relations between the Division of Physical Therapy and its alumni.
  • To promote the interests of the Division of Physical Therapy.
  • To enlist the influence and support of the alumni in joining with the University of North Carolina at Chapel Hill and the physical therapy profession in seeking the advancement of health services for human welfare.

Certification of Education Forms and Transcript Requests

Are you moving? Transferring your license? If you are moving or need to apply for a license in a new state, you will need to submit a request for an Education Certification to the University Registrar.  DO NOT submit forms to the division.
