Foundation for Physical Therapy and the UNC SPTA

on Friday, October 25, 2013

at the Chapel Ridge Golf Club in Pittsboro, NC.

12:00 p.m. Shotgun Start
Four-Person Scramble Format
(Full team not required for entry)

Entry Fee: $90.00 per player BEFORE October 1, 2013

$105.00 per player AFTER October 1, 2013

No entries will be accepted after October 10, 2013.
Entry fee includes box lunch, green fees, golf cart,
range balls, award ceremony and BBQ dinner.
