Registration is limited to a maximum of 30 participants. Incomplete documentation/payment will delay registration processing.
Payment
Payment of $695.00 (by check) is required for each participant.
Checks should be made payable to: Division of Radiologic Science
Seminar fee includes 2 1/2 days of instruction, access to state-of-the-art equipment, resources and course materials, one dinner, refreshments during breaks, and shuttle service to/from designated hotel and seminar site.

Cancellations/Substitutions
Payment is required in advance, but is refundable (less a $50 processing fee) if cancellation is made five weeks prior to seminar deadline (April 18, 2012). Cancellations after the deadline or failures to attend will not receive a refund. Substitutions are allowed, but as a courtesy we ask that you notify us so that we may prepare appropriate course materials.
