Application to the Minor in Speech and Hearing Sciences

We appreciate your interest in the Minor in Speech and Hearing Sciences. For information on the minor, see the advising sheet.

Instructions to apply for the Minor:

1. Download the application file (Only available after October 1).
2. Open the file in Adobe Reader or equivalent pdf reader (Preview in Mac)
3. Complete all questions  on the application
4. Save the document, adding your last name and first and middle initials to the beginning of the filename (if your name is Jane E. Smith, save the file as SmithJE-2011-SPHSMinorApp.pdf)
5. Close the file and re-open to be sure your responses were saved
6. Send the pdf file by email to Ina Diana (idiana@med.unc.edu)

Applications are due by 11:59 PM on November 1, 2012. Applicants will be notified of decisions by 11:59 PM on November 15, 2012.
