Submit 1 official transcript to the Allied Health Admissions Office . It is acceptable to wait until your fall grades are posted to do this. CB# 7120 Attn: Seletha Shaw
Submit 1 official transcript to the Division of Speech and Hearing Sciences - CB#7120 Attn: Ina Diana
Your recommenders can either submit their letters of recommendation online ( the system will automatically send them an invitation to submit it with a login and password) or they can mail them directly to me - CB#7120 Attn: Ina Diana
If your grades will not be posted by January 4, 2010, send me an unofficial transcript and have your institution send the official transcript with fall grades directly to me when they are posted – CB#7120 Attn: Ina Diana
If you are taking only pre-requisite coursework now as a continuing studies student, send a verification of enrollment form listing your coursework and then have your transcript sent when grades are posted.
