1 copy of official transcript with grades, from all post secondary work (post high school) including, community colleges attended, universities, and Study Abroad transcripts (signed across the back flap of the envelope and sealed). This applies even if credits earned at one institution appear as credits transferred on another institution’s transcript. An original transcript from each institution is always required. If grades or degree are not posted in time, send transcript to meet deadline, and send a follow up with grades.
3 letters of recommendation, signed across the back flap of the envelope and sealed (at least 2 from academic faculty). The signature requirement assures confidentiality. The signature on the back of the envelope, should be from the same individual that wrote the letter of recommendation. It is not required that you send hard copy letters if your “recommenders” do an online recommendation for you.
Personal Statement indicating reasons of interest in this field, potential specialty area of study, potential career objectives, and reasons for selecting UNC-Chapel Hill (2 pages maximum, double spaced, 12pt. font, 1” margins. Include complete name on each page.
