Office Hours
Monday, Tuesday, Thursday, and Friday:
7:00 am - 5:00 pm
8:30 am - 5:00 pm
:  8:00 am - 12:00 pm

Limited patient appointments available via appointment only from 5:00 p.m. - 6:30 p.m. on Monday, Tuesday, Wednesday and Thursday. Limited telephone service available during these hours.

**Please note that our doors open at 8am on Wednesday for those with Physical Therapy, Nutrition, or other appointments. Please arrive 15 minutes early for your appointment to allow enough time to complete any necessary paperwork and to update insurance information.  During your first visit, you will be asked to complete a patient medical history form.  You can print and complete this form in advance by clicking here.

If you are unable to keep an appointment please call us at 984-974-0210 as soon as possible to reschedule or cancel.  We request at least 24 hours notice, and in consideration of others, patients who repeatedly miss appointments without notice may be limited in their ability to schedule future appointments.

After Hours Care
For medical assistance after hours, please call 984-974-0210 and press 0 when prompted to speak to a UNC Healthlink nurse.  Healthlink nurses will work closely with UNC Family Medicine doctors on call to assist you.

Before Your Visit
Thank you for choosing the UNC Family Medicine Center! Prior to your visit, please complete the following:

New Patients:

  • Compile a list of your primary concerns to discuss with your health care provider during your visit.
  • Call your previous health care provider and obtain a copy of your immunization records to bring with you.
  • Bring your insurance card, photo id, all medications you are currently taking, and co-payment with you.  We accept cash, checks, debit, VISA, and MasterCard.
  • Bring your UNC Medical Record Number or if you do not have one, contact UNC Healthcare Patient Registration at 919-966-2555 to obtain one prior to your visit.
  • Complete a patient medical history form.  You can print and complete this form in advance by clicking here.
  • Arrive 15 minutes early to complete any necessary paperwork.
  • Review the Notice of Privacy Practices. You will be asked to sign a consent form on your first visit.

Returning Patients

  • Compile a list of your primary concerns to discuss with your heatlh care provider during your visit.
  • Bring your UNC Medical Record Number, insurance card, all medications you are currently taking, and any co-payments or balances due to UNC Health Care.
  • Arrive 15 minutes early to complete any necessary paperwork.

Lab Only Appointments

  • Open Monday, Tuesday, Thursday and Friday 8:15am-5:45pm and Saturday 8:15am-12:00pm; Wednesday 8:15am-5:45pm; and Saturday 8:15am - 12:00pm. Click here to make an appointment using My UNC Chart. 
  • All patients are required to check-in at the Front Desk on the day of your appointment.  Please do not go directly to the Lab.
  • Lab patients getting tested for glucose (blood sugar) and cholesterol (lipids) should not eat anything after 10pm the night before, or at least 8 hours prior to, your scheduled appointment. Patient should be fasting (no food or liquids) after 10pm.
  • Drink plenty of water before your visit.
  • Anyone who is given a lab order form by their provider can make a lab only appointment, and should schedule it before leaving the Family Medicine Center.  Walk-in appointments are still available but worked into available space.
  • Lab only appointments are scheduled for the hours between 8am and 12noon (no afternoon or Saturday appointments can be scheduled - walk-in only).  Lab appointments can only be scheduled by email once you leave UNC Family Medicine Center.  No call-ins please.  To schedule a lab only appointment, please contact the UNC Family Medicine Center using My UNC Chart  here.

Lab Results

  • You may access your available results using My UNC Chart here
  • You will receive a letter in the mail within 3 weeks of your lab visit if your results are normal.
  • If your results are abnormal or critical, your clinician will notify you.
  • If you have not received your results within 3 weeks, please call 984-974-0210 and give the name of the clinician and the date of your lab test. You will receive a return call.
  • If you have any other questions concerning your lab tests or results, please contact your provider using My UNC Chart here.  Please include your clinician’s name and date of lab service.
  • You will receive a separate bill from UNC Hospitals for lab services provided.

    Billing & Insurance

    Co-pays, co-insurance, deductibles, and money owed UNC Health Care for prior services are due at the time of service.  Deposits or prepayments are required for some medical, surgical, and diagnostic services.

    Financial counseling is available upon request.  UNC Health Care offers payment plans, a discount for self-pay patients, and a financial assistance ("charity care") program.  A financial counselor can explain these options and determine which programs may assist you.  For more information, please call 984-974-0210.

    The Family Medicine Center is a "participating" Medicare Provider and accepts Medicaid. Please bring your insurance or managed care card with you to each visit.
