reach adWhat is REACH?

The REACH program (Reaching out to Enhance the Health of Adults in their Communities and Homes) is an interdisciplinary team sponsored by UNC Family Medicine. The innovative program brings high quality care into the patient's home.  We extend the reach of UNC primary care from the hospital and clinic into a patient's home or assisted living community.

UNC Family Medicine's REACH program engages medical providers with expertise in home care, including board-certified geriatricians, nurse coordinators, clinical social workers and pharmacists.  Together as a multi-disciplinary team, we provide medically complex and at-risk adult patients with a high level of care coordination for their health care services, particularly around transitions of care.  We support patients throughout their life course and honor their values as they age.

Am I eligible to become a REACH Patient?

You are eligible to have REACH in your home:

  1. You are an established UNC patient.
  2. You are either homebound due to an illness or it takes exceptional effort to come to clinic appointments.
  3. Your established Primary Care Provider agrees to a REACH visit, or you do not have a PCP.
  4. You live within a 25-mile driving radius of UNC - Chapel Hill.

What happens once I become a REACH patient?older man

Once you agree to become a REACH patient, a REACH medical provider and nurse coordinator will come to your home for an initial visit.  The initial visit is 1 - 1 1/2 hours in length.  During the initial visit, the following concerns will be addressed:

  • medicine reconciliation
  • functional status evaluation
  • cognitive testing
  • social and family support evaluation

Based on the initial visit, a plan for follow-up visits is made.  Follow-up visits are conducted at a time of your convenience by one of our physicians or Nurse Practitioners.
