Before Your Visit: New Patients

NOW ACCEPTING NEW PATIENTS
The FMC is currently accepting new patients! Certain individual clinicians may be closed to new patients, but most are able to accept new patients. Please see the Appointments & Payment information for details.
Thank you for choosing the UNC Family Medicine Center! Prior to your first visit, please:
- Compile a list of your primary concerns to discuss with your doctor during your visit.
- Call your previous health care provider and obtain a copy of your immunization records to bring with you.
- Bring your insurance card, all medications you are currently taking, and co-payment with you. We accept cash, checks, debit, VISA, and MasterCard.
- Bring your UNC Medical Record Number or if you do not have one, contact UNC Healthcare Patient Registration at 919-966-2555 to obtain one prior to your visit.
- Arrive 15 minutes early to complete any necessary paperwork.
- Complete a registration form and bring it with you to your visit.

Review the Notice of Privacy Practices. You will be asked to sign a consent form on your first visit.

We are located at the corner of Manning Drive and 15-501 S. The Family Medicine Center is accessible from Chapel Hill and Carrboro via the U, RU, CM, and N bus routes. Parking for patients visiting our center is located directly beside the front entrance of the center.
