Description & Instructions

FINANCE Access Request
  • Complete this form to get access to finance functions in ConnectCarolina, including, but not limited to: Vouchers, Journal Entries, ePro, PCard, etc.
  • Send completed and signed forms to Lauren Gardner who will route for approval.
HR Access Request
  • Complete this form to get access to HR functions in ConnectCarolina, including but not limited to: PAAT, Basic Originator access, etc.
  • Send completed and signed forms to Matt Hawkins who will route for approval.
STUDENT Access Request 
  • Complete this form to get access to Student functions in ConnectCarolina
  • Send completed and signed forms to Lauren Gardner or Matt Hawkins who will route for approval.
SOM Infoporte Access Request
  • Complete this form to gain access to Infoporte and the SAS reports.  This form is specific to the SOM.
  • For HR access requests send to Matt Hawkins and for FINANCE access requests send to Lauren Gardner.
CHMED Business Unit Access Request
  • Complete this form to gain access to the CHMED business unit in both ConnectCarolina and Infoporte.
  • Send completed and signed forms to Alexandra Hayden and Betty Whichard.

How to agree to the Terms of Use

Follow this link to the Terms of Use – https://itsapps.unc.edu/ITSSelfStudy/CCTermsOfUse – then follow the steps below:

  • Login with your Onyen and password.
  • Click Policy.
  • Click Terms of use for University Administrative Systems Policy; this will launch a PDF of the policy.
  • After you read it, go back to the web page, click I certify that I have read and agree to the Terms of use for University Administrative Systems Policy.
  • Click the Confirm button.
