The CARE assignment – Timeline & Checklist

TBL during Orientation: learn about strategies to improve patient self-management of chronic disease including Motivational Interviewing and using the Biopsychosocial Model to understand social determinants of health.

Day 1-2 of clerkship: Meet with your preceptor and Identify appropriate patient for CARE assignment. Ask preceptor to identify questions to answer during consultation

Week 1-2  of clerkship – Review appropriate guidelines relevant to selected patient, meet with patient one or more times (in practice, in patients home, or in other location) and videotape one of those sessions using Flip camera where you demonstrate skills of assessing patient’s social context and/or motivational interviewing skills

Prior to Day Back session – Select 2-3 short clips on Flip recording that illustrate how you assessed patient’s social context and/or your motivational interviewing techniques. Clips should be short (2-4 minutes each and do not have to be perfect. Also come prepared to discuss guidelines.

After Day Back Session – Complete first portion of CARE consult note.  Briefly provide clinical summary for patient and outline questions preceptor had about the patient.  Reflect on what you learned from the encounter with the patient. Provide 3 prioritized recommendations for the patient using the questions posed by the preceptor, your synthesis of relevant guidelines and your understanding of the patient.

TBL #2 During Day Back Session – learn about concepts of population health and how to work with a registry

Week 4 of the clerkship – Define a population of patients in your practice with similar co-morbidities as your identified patient.  Define characteristics and outcomes of interest in that population and use registry function of practices HER or perform chart audit of at least 10 charts to assess outcomes belonging to this population.

Week 5 of the clerkship - Complete second portion of consult note. Describe population and outcomes you selected and present results. Reflect on implications of results and suggest one population based intervention that practice could implement for this population. Provide sufficient specifics and details that the recommendations to be helpful to the practice. Provide consultation note to preceptor and discuss (preceptor does not grade assignment). Also turn in note to campus director. Note is graded by campus director and typically is due at the end of week 5. Check with your campus director regarding exact due date.
