Diabetes Group Session

Appointments are available for established, returning patients. New patients who intend to begin receiving care from our clinic can be seen as well. Our program is ADA Self Management Education approved.

 Reasons for this Visit:
  • To provide you with the tools to help manage your blood sugar, blood pressure, and cholesterol
  • To provide you with education to help you reduce your risk of heart attack, stroke, kidney damage, and blindness
  • To provide an environment for sharing and asking questions about your diabetes care

What to Expect at this Visit:

  • You will spend about 4 hours in a group setting with diabetes educators and other people with diabetes
  • We will review multiple topics about caring for diabetes including diet, exercise, complications and overall health
  • You will learn a lot from others in the class
  • You will be expected to stay for the entire class

What to do to Prepare for this Visit:

  • Take all of your medications and check your blood sugar as usual on the day of the visit
  • If you have them, bring hearing aids or glasses
  • You do not need to fast (not eat) for this visit
  • Bring any questions you have or stories you wish to share
  • Bring a spouse or family member who helps care for your diabetes
