Diabetes Enhanced Care Visit
Appointments are available for established, returning patients. New patients who will begin receiving primary care from our clinic can be seen.
We look forward to working with you and your doctor to improve your diabetes control
Reasons for this Visit:
- To help lower your blood sugar, blood pressure, and cholesterol
- To review your medical history and make sure you have had the care and tests you need to help us keep you healthy
- Our goal is to reduce your risk of heart attack, stroke, kidney damage, and blindness
What to Expect at this Visit:
- The visit will take about one hour, you will see a diabetes specialist and educators
- We will review all of your medications and your blood sugar meter
- We will check your blood pressure, and when necessary, some blood or urine tests to check your diabetes control
What to do to Prepare for this Visit:
- Take all of your medications and check your blood sugar as usual on the day of the visit
- Bring all of your medications and your blood sugar meter with you to clinic
- You do not need to fast (not eat) for this visit
