Diabetes Enhanced Care Visit

Appointments are available for established, returning patients. New patients who will begin receiving primary care from our clinic can be seen.

Reasons for this Visit:

  • To help lower your blood sugar, blood pressure, and cholesterol
  • To review your medical history and make sure you have had the care and tests you need to help us keep you healthy
  • Our goal is to reduce your risk of heart attack, stroke, kidney damage, and blindness
 

What to Expect at this Visit:

  • The visit will take about one hour, you will see a diabetes specialist and educators
  • We will review all of your medications and your blood sugar meter
  • We will check your blood pressure, and when necessary, some blood or urine tests to check your diabetes control
 

What to do to Prepare for this Visit:

  • Take all of your medications and check your blood sugar as usual on the day of the visit
  • Bring all of your medications and your blood sugar meter with you to clinic
  • You do not need to fast (not eat) for this visit
