Tips on Preparing for a Trip to the Doctor

This is a check-list for clear communication at doctor visits.

  •  Ask these three good questions at every doctor visit:

  1. What is my main problem?

  2. What do I need to do?

  3. Why is it important for me to do this?

  • Bring a friend or family member you trust along with you

  • Make a list of your concerns or questions ahead of time

  • Bring all medicine with you to your appointment

  • Use one pharmacy to fill your medicines and ask questions at the pharmacy when filling new medicine

     

 

We have also developed (currently under construction as of 5.2009) a Patient Health Survey and Risk Assessment Tool that is intended to be completed before your visits.
