Tips on Preparing for a Trip to the Doctor
This is a check-list for clear communication at doctor visits.
Ask these three good questions at every doctor visit:
What is my main problem?
What do I need to do?
Why is it important for me to do this?
Bring a friend or family member you trust along with you
Make a list of your concerns or questions ahead of time
Bring all medicine with you to your appointment
Use one pharmacy to fill your medicines and ask questions at the pharmacy when filling new medicine
We have also developed (currently under construction as of 5.2009) a Patient Health Survey and Risk Assessment Tool that is intended to be completed before your visits.
