Diabetes Program

All clinic patients with diabetes are present in the registry and followed by the Enhanced Care Diabetes Program

Diabetes Program Overview: The Diabetes Program focuses on improving blood glucose and cardiovascular risk, including better control of hypertension, lipid treatment, smoking cessation, and use of aspirin and statins to prevent cardiac events.
Diabetes Protocols: Protocols/Algorithms and Decision Aids utilized by the Diabetes Program (ex. glycemic management, CV risk reduction)
Clinician & Planned-Care Tools: Visit Planner 5.0 (aka Yellow Sheet) used for all patients seen in the Internal Medicine Clinic
ADA-Approved Diabetes Self-Management Class: Group diabetes class for patients, offered twice a month (AM & PM)
Diabetes Eye Screening/Retinal Photography: We can take non-dilated retinal photos in clinic. This is a joint venture with UNC Ophthalmology.
