- Need a copy of the course description for each elective you are applying for
- UNC Extramural Course Form (Contact Leanne Shook for form)
- Signatures from your career goal advisor and by faculty member here at UNC that teaches a comparable course
Taking an International Elective
- Course description from your preceptor detailing your goals and objectives
- Sponsor letter from a UNC faculty member saying that he/she are willing to sponsor you
- UNC Extramural Course Form (Contact Leanne Shook for form)
- Travel Insurance (Contact Pattie Currie)
Taking an Elective at WFU, Duke or Brody
- Course Number
- Course Name
- Dates you want to rotate
