Transcripts
Transcripts come in two forms "Official" and "Copy". An Official Transcript is printed on special paper, has a picture of the student, the school seal and the Registrar's signature. A "Copy" of a transcript is a copy on plain white paper with a picture, but without a seal or signature. Any transcript released to a student or graduate, either Official or Copy, is stamped "Issued to Student". If the student or graduate wants a transcript released to her/him that is not stamped "Issued to Student", then it is placed in a sealed envelope. A transcript can be faxed, mailed or picked up from the office. Please allow up to three (3) business days to obtain an Official or a Copy of your transcript. Student Affairs must have a signed consent in order to issue either type of transcript. A request communicated by email, voice mail, or telephone is not adequate. The consent form must state the desired request (i.e. an Official Transcript or Copy), and where it should be sent or if it will be picked up, the student's ID# and year of graduation. Most importantly, the consent form must be signed by the student. Neither type of transcript can be prepared for a student if there is a "Stop" on the student's account.
Instructions for Ordering a Transcript
- Download, complete, print, and sign form and submit to the Office of the Registrar in person, fax, or by mail to 1001 Bondurant Hall, CB#9535, Chapel Hill, NC 27599-9535, Fax# 919-966-9930.
- Include $7.00 per transcript (this fee will be waived for students that are currently enrolled in the University of North Carolina at Chapel Hill, School of Medicine)
