Written Request
By law, we are required to have your written consent for any request that you ask of our office. We have a consent form in 1001 Bondurant Hall, which makes it easy for you to request anything from your file such as a letter of good standing and enrollment, letter of expected graduation, letter certifying that you are a graduate, an official and unofficial transcript and anything else that you may need the Registrar’s Office to do for you. If you are on an away rotation or out of town and find that you need something from the Registrar’s Office, you can send us a letter including your written signature. We cannot accept a phone message or email as a written consent.
