Exit Checklist

By the end of the clerkship, each student must submit:

________ 1 Observed H&P evaluation form (observed by an attending, chief resident, or fellow, NOT by resident/intern)

________ 6 H&Ps signed by attending (including the H&P for formal evaluation by Dr. Klipstein)

________ 1 H&P for formal evaluation

For inpatient month 1:

________ at least one faculty evaluation of the student, and

________ at least one resident evaluation of the student, and

________ at least one intern evaluation of the student, and

________ one evaluation of each of the attendings, residents and interns

For inpatient month 2:

________ at least one faculty evaluation of the student, and

________ at least one resident evaluation of the student, and

________ at least one intern evaluation of the student, and

________ one evaluation of each of the attendings, residents and interns

All evaluations must be submitted to the clerkship office within 2 weeks after the last day of the clerkship. Evaluations turned in later than 2 weeks after the last day of the clerkship might not be accepted.

Students must also complete:

________ UNC Medicine Exam (at each clerkship site at end of 1st rotation)

________ NBME Subtest in Medicine (in Chapel Hill for all students at end of clerkship)

________ Clerkship Evaluation (completed the last day of the clerkship)

________ Clerkship Patient Log (on one45)
