Patient Information Clinical Programs

To refer a new patient to UNC Neurosurgery, please complete the
New Patient Appointment form (.pdf) and FAX it to the UNC Neurosurgery office at 919-966-6627. Along with the completed New Patient Appoinment form, please also include any pertinent medical records and/or imaging, detailing the patient's medical history and diagnosis.

For new patients and referrals, call 919-966-1374.

*On the day of the appointment, it is imperative for the patient to bring with them the following:
- Insurance information
- A list of all medications with doses or actual medicine bottles
- All imaging studies (either on a CD or hard copy x-rays)
IMPORTANT NOTE: If the patient comes to his/her appointment without the necessary imaging studies, the physician will be unable to see the patient, and the appointment will need to be rescheduled.  This is to ensure that the physician has all the necessary information to make the visit a productive one.

Established patients needing to schedule a follow-up appointment or cancel/reschedule an existing appointment in the Neurosurgery Clinic should call 919-966-8810, Monday - Friday between the hours of 8:30-4:00pm.
