How to make an appointment

 

Patients are seen on a referral basis from other physicians or you may call without a referral.  All new patients to UNC or those that have not been seen in over one year should update their records. This will make your visit easier by saving time on the day of your appointment.

Please have the following information available

  • Full Name
  • Social Security Number
  • Date of Birth
  • Address
  • Phone Number
  • Insurance Information
  • Referral or authorization number (if you have a managed care plan)
  • Referring physician's name and phone number
  • UNC Hospitals patient number (if you have been seen at UNC before)
