To be seen in our clinic, you can:
- call to make an appointment directly,
- you can have your primary-care physician make a referral for you, or
- you can click here to request an appointment.

• Full Name • Insurance Information • Referral or authorization number (if you have a managed care plan) • Referring physician's name and phone number • UNC Hospitals patient number (if you have been seen at UNC before)

To get the most out of your first appointment, we ask that you do a few things in preparation for your visit:
- Please obtain your pertinent medical records from your other health care providers.
You can have your health care providers send those records directly to our clinic or they can send them to you directly.
