• call to make an appointment directly,
• you can have your primary-care physician make a referral for you, or

To get the most out of your first appointment, we ask that you do a few things in preparation for your visit:

1. Please obtain your pertinent medical records from your other health care providers. 
   
   You can have your health care providers send those records directly to our clinic or they can send them to you directly.
