Access Cards

 

The Planning Office provides electronic door access cards which are programmable, allowing various levels of access to all School of Medicine facilities.  While there is no limit to the number of facilities on your individual card, you must obtain the appropriate authorization.  Note: Access to any animal facility must be authorized separately by the Division of Laboratory Animal Medicine (DLAM).

New employees must have a PID before any card can be processed.

Please inform us immediately (843-3787) if your card is lost or stolen and we will disable your card. This will prevent unauthorized use of your card.

Until further notice, the Photo ID Office, located in 041 MacNider, will have the following hours of operation:

MONDAY-FRIDAY 

8:30 AM - 4:30PM

OBTAINING YOUR ACCESS CARD

If you are a new employee, or you need to have your access card changed, follow these instructions:

  • Complete a card access authorization form by clicking on Card Access Authorization on the left. 
  • Have the appropriate person sign the form
  • You must bring the form to 041 MacNider for processing;
  • There will be a $15 processing charge for all replacement badges.
  • Do not fax forms if person is getting a badge for the first time. (We do not have a fax machine in the Photo ID office)
