When a little more help is needed.

The Case Management team provides community-based services for individuals with serious mental illness who need additional assistance to improve well being and to connect to helpful care and services. Working with individuals and their families, the team assesses needs and collaborates with clients to develop a plan that focuses on the client’s goals.

The evidence-based program provides several different types of assistance including:

  • Develop skills that will enhance ability to live more independently
  • Linking with needed services and resources
  • Provide support and coordination of care to maintain engagement in treatment and enhance recovery

Clients in the program have access to community resources as well as to multiple innovative programs at the Center, including, supportive employment, horticulture therapy at the Farm at Penny Lane, UNC PAWS, the Brushes with Life Arts program, and various wellness activities. Additionally, clients may participate in the Integrated Care program that includes a primary care clinic and dental screenings at the Carr Mill Offices.

UNC CEMCH clients may be referred, and they will be screened to determine eligibility.
