The Center’s Community Support Team provides enhanced services to individuals with severe and persistent mental illness coupled with medical conditions. Individuals work with a comprehensive treatment team, including a psychiatrist, master’s level clinicians and paraprofessionals in conjunction with primary care physicians and specialists to integrate mental health and physical health care. Treatment often focuses on health and wellness.
The treatment team provides medication management and individual therapy, along with case management services that assist individuals in maintaining psychiatric recovery and independent living.
Therapy modalities include Cognitive Behavioral therapy, Dialectical Behavioral therapy, Motivational Interviewing, Illness Management and Recovery, Wellness Recovery Action Planning and group therapy.
Medical necessity must be demonstrated prior to service authorization. Individuals who meet criteria for this service have an Axis 1 diagnosis of schizophrenia, other psychotic disorders, or bipolar disorder.
Individuals qualifying for these services would likely have a history of:
- psychiatric hospitalizations
- a co-occurring diagnosis of substance abuse
- legal issues and/or a risk of homelessness coupled with medical conditions
- trying lower levels of care and having difficulty with compliance
