The following criteria must be met in order to receive services at Wake STEP:

  • Wake County resident
  • 18 years old or older
  • Diagnosis of bipolar disorder, schizoaffective disorder, or schizophrenia

In order for us to begin review of your referral, we need the following items:

  • Fully completed referral form
  • Medical records from your most recent and/or current mental health provider/inpatient facility, including most recent labs, if applicable. We need your records sent to us in order to begin review of your referral. You are responsible for contacting your previous or current provider to request the records to be sent to our office. Your provider can fax your records to: 919-445-0405 OR mail to 401 East Whitaker Mill Road, Raleigh, NC 27608.
    • If no records are received, your referral will not be reviewed and will be closed in 60 days.

What happens once we receive your referral form and records?
Once we received your completed referral packet, it will be reviewed by clinic management and a determination will be made. You will be contacted via mail and/or phone with the disposition.

What if I need services immediately?
If you should need services from another provider, please call Alliance Health at 1-800-510-9132.  If you have a mental health emergency, please call Wakebrook Crisis and Assessment at 984-974-4830 or 911 for an immediate emergency.

For all new referrals, please use the appropriate referral form listed below, and fax to 919-445-0835 addressing to the attention of ‘Referrals’. Our referral specialist will contact you within 72 business hours to conduct pre-screening for program eligibility. If you are a provider, and are unsure about whether to request a consult or make a referral, please call 919-445-0838.

If you are experiencing a financial hardship surrounding affording the cost of treatment, our staff will provide financial counseling and referrals for assistance on an as needed basis.

We are in-network with most insurance plans.

Patients are eligible for Enhanced Primary Care if they have a documented severe mental illness diagnosis and are referred by their current psychiatric provider or team. SMI diagnoses include schizophrenia, schizoaffective disorder, bipolar disorder, major depression with psychotic features, psychotic disorders, etc. This process helps REACH verify each patient’s SMI diagnosis but most importantly ensures the availability of psychiatric medication management and/or supportive therapy. REACH works in partnership with psychiatric providers but does not routinely prescribe antipsychotics and other important psychiatric medications through primary care. The REACH Referral Form can be faxed with records and release of information to the intake team at 984-974-4920. REACH is currently scheduling new patient appointments 1-2 weeks after receipt of eligible referrals.
