Accounting of Disclosures Requirements

UNC HCS staff who disclose PHI for research purposes ARE NOT required to maintain an accounting of disclosures under the following circumstances:

  • Disclosures with written Authorization of the patient

  • Disclosures of Limited Data Sets

  • Disclosures of De-identified information
