Make an Appointment
Please fill out this form to make an appointment at the SHAC Medical Clinic. The Clinic operates on Wednesday nights with appointments only being made between 5:30 and 8 p.m.
There will be NO clinic on the following dates: October 31, November 21, December 19, December 26, January 2.
