SHAC Outreach

Who are We?

SHAC Outreach is devoted to serving the underserved community in the Research Triangle Area including the Latino population in Chapel Hill and the homeless in Durham. Our mission is to provide the best patient-centered care for those who otherwise lack access to healthcare services and to ensure that our patients have the means to obtain follow-up care in a way that meets their financial needs. Our program includes two major services: we provide as-needed health screenings for events such as fairs and festivals. Secondly, we have established partnerships with both the St. Thomas More Catholic Church as well as Urban Ministries Durham to provide walk-in clinics on a monthly basis.

What Services do We Provide?

For our as-needed healthcare fairs, we provide services including blood pressure, blood sugar, body mass index (BMI), cholesterol, HIV, and hepatitis C screenings. If you would like to request for SHAC Outreach to participate in one of your events, please click here.

We also provide monthly walk-in clinics at both the St. Thomas More Catholic Church (STM) and at Urban Ministries Durham (UMD). At STM, we offer blood pressure and blood sugar screenings as well as medication counseling. We also have established a program called “Medication Therapy Management” (MTM) in which a pharmacy student and a pharmacist goes over medication regimens to make sure there are no drug interactions and no side effects.

At UMD, we offer blood pressure and blood sugar screenings as well as medication counseling. Additional to these services, we also give free hepatitis C and HIV screenings. At both STM and UMD, we have established partnerships with many clinics including SHAC Clinic, Samaritan Health, and Lincoln Health in case patients need follow-up care.
