For New Patients

Referrals are accepted from physicians only.  Please have your doctor's office contact us at 
919-966-4389 and then press #1.  Your doctor can also fax your referral and records to 919-966-0369.  We will then contact the patient to schedule their appointment. 

For Return Patients

Please call our office to schedule your trauma or general surgery follow-up appointments at 
919-966-4389 and then press #1.

For Physicians

Please fax a new patient referral with the pertinent notes for your patient to 
919-966-0369. 
Once the referral is triaged by our nurse we will then contact the patient directly to schedule the appointment.

Transferring Trauma Patients to UNC Hospital

Please contact the UNC Transfer Center at 919-843-1920.  They will help facilitate a patient's transfer to UNC Hospitals.
