Making an Appointment

You can make your own appointment for either transplant specialty by calling 1-888-263-5293. Please request additional information on the referral process when you call. Clinic evaluations are done on Mondays from 8am to 4pm and on Thursdays from 1pm to 4pm. There is an additional kidney transplant clinic on Wednesday's from 9 am to 12 pm. When arranging an appointment, individuals or referring physicians should provide the following information:

• The patient's full name
• Address
• Phone number
• Date of birth
• Social security number
• Diagnosis or explanation of the current medical problem

• Insurance coverage

• For the referring physician, we need:
