Learning Objectives: Increase competence in obtaining a detailed history and performing comprehensive physical examinations and synthesizing the information into a differential diagnosis and to begin developing diagnostic and inpatient services including cardiovascular disorders, community-acquired infections, complicated multi-system disease processes, diabetes, and other endocrine disorders, and its related complications. The acting internship is under the supervision of upper level medical residents and attending faculty.
Learning Activities: Acting interns will be assigned to one of the general internal medicine in-patient services (B-Service) which admits patients from the internal medicine clinics or the emergency department of Moses Cone Hospital. Students will be on call, under the supervision of an upper-level medicine resident. Teaching rounds will be held on a daily basis, emphasizing building sound clinical skills. Noon-time conferences are held on Monday through Thursday, and Medical Grand Rounds on Friday mornings. Acting interns will have call responsibility q:4. All conferences, seminars, and rounds are part of the expected day.
Evaluation: Attending will provide written and verbal evaluation at the end of the rotation. Verbal feedback will also occur halfway through the rotation.

Learning Objectives: This rotation emphasizes primary patient care involving a blend of in-patient and out-patient experiences. Students function as acting interns on one of five ward services with primary responsibility for the care of assigned patients under the supervision of a resident and an attending. Required conferences include morning report, daily subspecialty lectures, weekly grand rounds, and a monthly ECG conference. Acting interns attend the general medicine out-patient clinic two times each week. This provides a means of developing skills for the efficient management of acute or chronic as seen in the office of a general internist.
Special Notes: Orientation is crucial to the rotation/your experience in this department. If you miss orientation, you will have to reschedule the entire rotation.

Learning Objectives: Acting interns function as primary physicians for in-patients with both acute and chronic medical problems in the context of an urban medical center. Medical problems are complex and diverse in this primarily indigent patient population and opportunities for direct hands on patient care are extensive. This is a busy rotation in which AIs follow patients on the medical wards. Students work with UNC interns, residents and faculty, as well as with practicing physicians in the Raleigh community. Didactic lectures, conducted by the chief resident and faculty are designed to cover core topics in clinical medicine. Hospital services such as 24 hour blood drawing teams and patient transport services, minimize the time spent doing "scut-work" and maximize learning opportunities.

Fourth year students serve as acting interns to patients on the acute medicine inpatient service who are suffering from both acute and chronic medical problems. During the four week rotation, fourth year students may split their rotation with two weeks on the resident inpatient critical care service and two weeks on the resident inpatient general internal medicine service; or a fourth year student may elect to spend the entire four weeks with the resident inpatient critical care team or on the inpatient general medicine service. If only one acting intern/fourth year student is assigned during the rotation, he or she will preferably spend their entire four-week block on one or the other of these two inpatient services. This elective opportunity allows fourth year students to learn and experience the responsibilities of caring for patients directly under the supervision of a senior resident and the medicine attending. The fourth year student is strongly encouraged to attend all Internal Medicine resident teaching conferences and Internal Medicine Grand Rounds.

Learning Objectives: Students serve as acting interns in Nash General Hospital under the supervision of clinical faculty who are in the private practice of Internal Medicine in Rocky Mount. Students are assigned to selected patients who are hospitalized under the care of the responsible clinical faculty member, and other hospitalized patients who are of particular teaching value. For these patients, students are given appropriate responsibilities for ordering diagnostic evaluation, writing orders, etc. Opportunities are also available for students to actively participate in the emergency services and to attend grand rounds and other educational conferences at the hospital. Learning Objectives: Students participate in both didactic and clinical settings pertinent to obstetrical and gynecological physiology and pathophysiology. History and physical examinations are performed on ambulatory and hospitalized patients. Students formulate plans for the care of obstetrical patients, monitor the maternal and fetal patient during labor, and participate in her delivery. With the gynecological patient, students are involved in the pre-operative evaluation, surgery, and post-operative care, encouraging total continuity of care of the patient. Learning Objectives: This program is designed to provide an advanced experience in gynecologic surgery and high risk obstetrics to students interested in obstetrics and gynecology as a career. The rotation will not result in many additional "routine" deliveries as these are the legitimate provinces of the third year student. Acting interns can expect to improve their skills assurgical assistants and/or become involved in the management of complicated obstetrics patients. Depending upon the previous experience of the AI and the total number of students on rotation, we can, at times, offer a focus in sub-specialty practice or office practice. Primary Learning Objective: Acting Interns serve as members of the outpatient and inpatient care team at the NHRMC Residency Program in Family Medicine. Acting Interns will round on all inpatients, take primary responsibility of assigned patients, actively participate in teaching rounds, attend scheduled conferences, take night call (home call approximately one to two nights per week as well as one weekend during rotation), and provide acute and continuity care to patients at Coastal Family Medicine. Due to the volume of patients in both the inpatient and outpatient settings, students will generally be exposed to obstetrical care, neonates, pediatric patients, adult and geriatric populations, nursing home patients, and rehabilitation and other hospital consultations and will receive excellent exposure to the common medical problems encountered by family physicians in both inpatient and outpatient realms.
Learning Activities: The student will round on the inpatient service in the mornings with the hospital resident and attending. As a family medicine service, the student will have the opportunity to work with a variety of patients - the service averages 4-6 adult patients and in addition includes several new mothers and babies, unassigned newborns, and hospitalized children. In the afternoons, the student will work in the clinic setting seeing acutely ill patients with the Family Medicine Resident and Attendings.
Evaluation: Students will be evaluated using the AI evaluation form. The evaluation will be completed by the Faculty preceptor, and will be based upon presentations, participation, and skills in managing patients on the Family Medicine service.

Learning Objectives: Students have the opportunity to function as first year residents on the pediatric ward of a moderate size community hospital. They will take on greater responsibility than clerkship students for hospitalized patients. They also get the unique opportunity to assess children in the emergency department. Procedures are supervised and orders countersigned by residents. Teaching rounds with the attending are conducted daily. The Selective is ideal for students interested in a pediatrics or family medicine residency, as well as other specialties that involve the care of children.

Learning Objectives: The student will be able to evaluate children (infant to 18 years), write admission orders, develop an assessment and plan, make daily decisions, regarding a wide variety of conditions necessitating an inpatient stay. They will learn to present patients in a comprehensive yet succinct manner. They will learn to search the literature for evidence-based practice.
Learning Activities: The student will carry an average of 6-8 patients, admitting 3-5 on a call night. They will have an average of 7 call nights. They will attend morning chief rounds, morning report, attending rounds, radiology rounds daily. They will attend psych rounds weekly. Lunchtime departmental conferences and weekly Grand Rounds. They will have primary responsibility for their patients including procedures, notes, communicating with parents and consultants.
Evaluation: The student will receive mid-month feedback and end of month feedback from the inpatient chief and attending. They will have an exit interview with the acting intern coordinator. Their evaluations will be compiled from direct observation of interview, physical exam skills; oral presentations, written notes. They will be expected to give a short "chalkboard talk" to the team. They will be evaluated by the attending, chief supervising 2nd year residents. Information from support staff and patient families will be included.

Learning Objectives: WakeMed Health & Hospitals is a 593 bed hospital in Raleigh. There are 25 beds for children on a combined Pediatrics-Surgery ward. There is also an 8 bed Pediatric Intensive Care Unit. Pediatrics house staff from UNC-CH rotate through this service and acting interns are part of this team. Acting interns are responsible primarily for patients on the ward. This Selective is designed to provide exposure to the types of illness seen in a community hospital while providing a close association with faculty preceptors and practicing physicians. Learning Activities: I. Daily Schedule

0700* - Round on inpatient service with team. We will meet in the small conference room on the 7th floor of the Hospital, almost opposite the call rooms. (Entry code is 4-5-3-2). You should have pre-rounded on your patients by taking an interim history, performing a focused physical examination and gathering any outstanding laboratory data. In addition to patients you are following, new patients that may have been assigned should be seen. After inpatient rounds, you should complete your notes.
08h30 - Morning Report. This is presented by the Resident Teams in the Resident Conference Room on the 7th floor. (Entry code is 3-2-1). You will also have the opportunity to present patients at Morning Report.
09h00 - X-Ray report (This is on the 4th floor in the Pediatric Radiology Annex Resident Teams review films from 09hOO-09h15 approx, Hospitalist Teams review films from 09h15)
09h30 - Rounding on patients.
12h30 - Lunch Conferences with Faculty (Scheduled Monday thru Friday, refer to departmental conference calendar for exact topic and time).
13h00 - Follow afternoon schedule. You will be assigned to new admissions to the Team or to self-study. Self-study is not free time. You must be available during this time for new admissions to the inpatient units. If there are no new admissions, you should be engaged in self-study.

Pediatric Grand Rounds
This is held each Wednesday morning at 7:30am, attendance is expected.

Evenings - You will be assigned overnight call, this will work out to every fourth night. The call nights should be scheduled with your peers and the Supervising/Chief Resident.

Weekends - Weekend responsibilities; You will be expected to take week-end call (l-in-4 as part of this rotation), i.e. an overnight call on a Friday, Saturday and Sunday though these will not be consecutive call nights.

Family Care Rounds
This is held each Thursday at 1:00 pm in the PICU conference room.
There is also an informal conference on Monday at 11am in the 7B conference room.

II. Inpatient Responsibilities
A. The inpatient service is made up of the Faculty attending(s), Nurse Practitioners, Assistant-Intern medical students. A third year resident elective may occasionally be part of the team.
B. New patients admitted during the day should have histories taken and physical examinations performed. These should be documented on the white "History and Physical" sheets provided to you (These are available on-line also). In addition to evaluation of chief complaint, focus should include pediatric issues such as growth, development and immunization status. Each patient's weight, height and head circumference <2 years) should be plotted on the growth charts available on the floor. The attending/resident will review each new admission with you.
C. New patients who were evaluated by the overnight team do not require repeat history and physical examination, however clarifications of points of History and focused Physical Exam may be obtained. Instead, these patients should be divided among the members of the team. The Attending, may assigning patients to you. These patients' initial history and physicals, any laboratory data, interventions and subsequent hospital course should be reviewed.
D. Follow-up on each patient's progress on a daily basis. This includes gathering further history, examining patients daily, (more frequently if needed), and checking laboratory data in a timely fashion. You should also follow up on any teste s) that have not been reported prior to your patient's discharge.
E. Documentation of this follow-up should take place via daily progress notes. Notes should be in the SOAP format and be problem-oriented.
F.The medical student should be involved with discharge planning, including parental education and interaction with patient's primary physician. We always generate and fax discharge information to the referring MD when a patient is discharged. We often follow up with a call to the referring physician's Office.
G.All patients should be seen prior to morning rounds. Notes may be written after rounds
H.Presentations at rounds should be in one of two formats.
i) New patients (including all patients who have never been presented at morning rounds) should have the following information presented in the following order:
a. Initial History
b. Initial Physical Examination, including HeightlWeight/Head Circumference data c. All Laboratory DatalRadiology Results/etc. If asthmatic PEFRs; if dehydrated Ins & Outs; if diabetic blood sugar profile; if febrile, fever profile etc
d. Initial Assessment and Plan
e. Hospital course, including today's physical examination, since initial assessment.
H ii) Established patients should have the following information presented in the following order:
a. A one-line summary of the patient. This should include the age and the working diagnosis. DO NOT REVIEW THE INITIAL HISTORY ON ESTABLISHED PATIENTS! For example, "This is a 3 month old with brochiolitis" is adequate.
b. A summary of their course since the previous days rounds. For example: "The patient was weaned from their oxygen tent overnight, has continued to feed well and has no new problems."
c. Vital signs (HR, RR, T max in last 24 hours and daily weight (if available) and physical examination. If asthmatic PEFRs, if dehydrated Ins & Outs, if diabetic blood sugar profile, if febrile, fever profile etc
d. New laboratory data.
e. Your assessment and plan for the day.

III. Conferences
A. Attend any 12h30 conferences scheduled.
B. Attend Grand Rounds Wednesday at 07h30.
IV. Self-Study
A. You should read more in-depth on topics related to the patients you are involved with. You can find sources with greater detail than the provided text in the library.
V. Evaluations
A. Student evaluation forms are provided to you by the UNC Medical School.
B. Your evaluation will be reviewed with you at the end of your rotation.
C. You will have an evaluation also at the mid point of your rotation with us.
VI. Absence
If you will be absent on a particular day for any reason (ie. illness, personal day, vacation), you must inform Dr. Tynan and/or Dr. Noonan prior to your absence. You should also leave a message with Kathy Adams, (704-355-3156).

Learning Objectives: Students will work as clinical clerks or externs under the direct supervision of house staff and attending staff, the latter including a full-time Director of Surgical Training. Work will consist of doing histories and physical examinations upon and of observing and assisting with pre-operative and post-operative treatment of hospitalized patients, and also of acting as second assistant during operations on these patients. Students also will work in the clinics and will be placed on a night call roster to assist house officers of nocturnal surgical activities. The practical experience thus gained is complemented by regularly scheduled formal conferences and lectures and by informal person-to-person instruction and discussion as the opportunities for the latter present themselves. Learning Objectives: 1. This program is designed to give one medical student the opportunity to directly participate in the total surgical management of the sick patient with an experienced surgical team. The person need NOT be surgically oriented.
2. The student will manage patients and operate with skilled surgeons and housestaff in a busy practice entailing ambulatory general surgery and inpatient trauma/critical care and general surgery.
3. The student will participate in patient care planning with nurses, physician assistants and social workers in order to develop total resource skills.
4. The rotation is complimented by didactic sessions with members of the house staff and teaching faculty.
Learning Activities: 1. Daily team patient care, rounds and conferences.
2. Emergency department and main OR surgical management (includes trauma).
3. Night call with senior residents and in house faculty, no more than one in four.

Learning Objectives: Students serve as acting interns and as members of the Family Medicine inpatient team. They round on all inpatients, take primary responsibility of assigned patients, actively participate in teaching rounds, attend scheduled conferences, and take night call with upper level residents(usually every 4th night). Inpatients for this service are generated from the residency's Family Practice Center, private community family practice offices and unassigned patients. Because of the primary care patient base, students receive excellent exposure to the common medical problems requiring hospitalization.

In order to accommodate students interested in future family medicine residency training the Moses Cone Family Practice faculty asks that those students not planning on such residency training, schedule their acting internships at our residency for periods 8, or 9.

Learning Activities: Daily activities include rounds, conferences, patient work-ups, supervised procedures and other in-patient management. AHEC 440 Acting Internship in Family Medicine, Carolinas Medical Center

Learning Objectives: As members of the in-patient team, students care for their own panel of patients from admission to discharge. Under the supervision of family medicine attendings and upper level residents, the AI will be encouraged to make independent assessments and plans during evaluations in the emergency department, family practice clinic and in-patient service. The students will participate on rounds and take call with a resident approximately six nights during the 4 week rotation. Out-patient experiences in the Family Practice Center in the afternoons are included one afternoon per week. Students will have the opportunity to care for patients of all ages, including OB/newborn, pediatric and adult patients.

Carolinas Medical Center is a busy regional medical center in a thriving metropolitan area. Charlotte AHEC is offering new housing in a gated community with full amenities including high speed internet, pool and a fitness center. This attractive housing is just minutes from the hospital and uptown Charlotte. Letter of intent, copy of transcript and housing application are required in advance. Priority is given to students who are interested in family medicine.

Learning Activities: The students will join the inpatient family medicine residency team. The will be assigned patients at the level of the interns on the team. The students will take call every 4th night on average. Daily activities will include rounds (notes and co-signed orders), new admissions and discharges (including dictation), procedures under supervision on the patients they follow and case presentation during morning report at least once during the rotation. The students will attend resident lectures/workshops as they are available during the rotation.

In order to accommodate students interested in future family medicine residency training, the faculty of Cabarrus Family Medicine Residency asks that those students not planning on such residency training to schedule their acting internships at our residency in the latter half of the academic year.
