PSYY 403 Child Psychiatry

Learning Objectives: 1) Develop strategies for screening and disposition of out-patient emergency or semi-emergency child and adolescent patients.
2) Develop skills in outlining and effecting diagnosis and treatment of children and adolescents in an in-patient psychiatric setting.
3) Gain proficiency in psychiatric consultation to a pediatric in-patient population with illnesses such as cancer, burns, renal failure, and cystic fibrosis.
4) Develop an awareness of the complex ethical issues in child and adolescent psychiatry.

The Child Psychiatry elective will be spent in out-patient and in-patient settings. The student will follow a child or adolescent on the inpatient unit at UNC in an acting intern position during their month. They will see this child at least four out of five days a weeks and be a part of all decision-making in their treatment plan. Out-patient settings will include UNC, Dorothea Dix, and the Wake Behavior Clinic. The UNC experience will consist of 1 1/2 hour diagnostic screenings that occur 4x each week. These are done with the child psychiatry fellow and supervised by the attending. The student will participate in the interview process and learn methods of diagnosis and treatment of children and adolescents. Lastly, the experience will include two half-days of closely supervised psychoparmacology child clinics at UNC and psychopharmacology clinic at Wake Hospital. The student will be expected to attend all didactic (about 6 hours per week). The student will be at Dorothea Dix in Raleigh on Wednesday morning for the extended case conference.
Learning Activities: The student will be expected to attend all of their learning experiences as described above.
Evaluation: 1) The students will be evaluated by the attending with whom they worked the most closely after that attending consults with others who have had contact with the student.
2) The student will be observed directly in the interview process and in the treatment process with children and adolescents.

PSYY 404 Consultation Psychiatry

Learning Objectives: This experience emphasizes learning to recognize and manage the psychological complications of medical and surgical illness. Patients with organic brain syndromes, classical "psychosomatic" illnesses as well as major psychiatric disorders are seen on the wards of the University of North Carolina Hospitals.
Learning Activities: Individual supervision of patient care is provided by attending staff and advanced psychiatric residents. Students attend the rounds and conferences of the Consult service and are encouraged to make at least one clinical case report at a scheduled teaching conference. Full time divided between wards, clinic, and conferences.
Evaluation: Dr. Gala will consult with other team members, attendings, residents, and the administrative coordinator to make the formal evaluation.

Learning Objectives: 1. To gain an awareness of the complex issues involved in the diagnosis and treatment of patients admitted to a state psychiatric facility.
2. To develop skills in managing the care (psychiatric and medical) for patients with a variety of severe psychiatric illnesses
Learning Activities: Depending on the patient population, students will be able to select the types of patients they wish to work with. Students will participate in:
· Conference: 2 hours per week
· Ward functions (including rounds, treatment team meetings, patient interviews, developing treatment plans and discharge plans): 32 hours a week
· Admissions/screening: 8 hours
· Library: 4 hours
· Read: 4 hours
Evaluation: Students will be evaluated:

Learning Objectives: 1. Recognize the interface between psychiatry and the criminal justice system.
2. Develop an awareness of the role of a forensic psychiatrist including conducting pretrial evaluations, reporting on competency to stand trial and criminal responsibility, and providing short-term psychiatric treatment.
Learning Activities: There are not service responsibilities for this rotation.
Students will:
· Work with a forensic psychiatrist doing pretrial evaluations, reporting on competency to stand trial and criminal responsibility.
· Participate in ward activities including rounds each morning, treatment team meetings, interviews.
· Attend the Forensic Case Conferences
· If available go to court and observe testimony of the psychiatric expert witness.
Evaluation: Students will be evaluated:

· Develop an appreciation of the role of neurobiological research in the treatment of severe
neuropsychiatric disorders.
· Understand the protocols involved in the research studies including one comparing the
efficacy of atypical antipsychotics in the treatment of refractory schizophrenic patients, a
longitudinal study of first episode psychoses, and an evaluation of new antipsychotic drugs.
Learning Activities: Students will:
· Be exposed to neurobiological research in a psychiatric setting.
· Work Individually with patients with severe neuropsychiatric disorders on a 16-bed in-patient unit devoted to psychopharmacology and clinical neuroscience research.
· Participate in routine ward activities (rounds, treatment planning sessions)

Learning Objectives: As a result of this experience students should:
1. Enhance skills in assessment, diagnosis and treatment planning in a variety of ambulatory care settings (ER, clinics, special populations)
2. Greater understanding/experience with the strengths/assets of multi-disciplinary team in providing community-based outpatient psychiatric care.
3. For interested students, opportunity to expand knowledge of psychosocial/medical treatment of substance abuse disorders.
4. Hone patient interview and presentation skills. It is the students' responsibility to arrange their housing.
Learning Activities: 1. Students will attend conferences, grand rounds, and/or "field trips" almost daily according to their individual schedules.
2. Hours vary with the service to which student is assigned. There is no required call; however, students are strongly encourage to take advantage of the opportunity for some "after hours" experience in our busy 24-hour psychiatric emergency room.
3. Student attends rounds, treatment team meetings, and staffings as their service dictates
Evaluation: Evaluation of student is by direct observation of the students work by attending physician(s) and generally incorporates input from other physicians and allied health care professionals who have first-hand knowledge of the student's work.

PSYY 436 Acting Internship in Psychiatry, UNC Hospitals

Learning Objectives: 1. Enhance psychiatric interview skills in emergent and non-emergent situations
2. Increase knowledge of psychopathology and appropriate treatment interventions
3. Develop supportive therapy skills and enhance understanding of psychodynamic issues
4. Gain additional knowledge of inpatient treatment for specific groups of patients depending on unit selected (Crisis Stabilization, Geropsychiatry, or Psychotic Disorders)
Learning Activities: The student will function as an intern under the direct supervision of the ward attending and with the assistance of on-unit residents. The student will follow 3-4 patients and be the team leader for these patients on rounds and at team meetings. The student will have an opportunity to enhance knowledge and skills learned during the third year and will be expected to be involved in all aspects of a patient's care including discharge planning, family meetings, and medical and psychiatric treatment. Emergency evaluation skills will be enhanced by 1/2 day per week in the Emergency Walk-In clinic working with the 2nd year resident on call. (Evening call one night per week (until 10 pm) can be substituted for the Walk-In experience by request). The student will be invited to attend ongoing case conferences for the 3rd year students as well as the Chairman's Rounds and Grand Rounds.
Evaluation: The student will be evaluated by the ward attending, in consultation with any residents the student has worked with.

PSYY 437 Crisis Emergency Services

Learning Objectives: 1. Familiarize student with emergency psychiatry.
2. Work with multidisciplinary team to make diagnosis and formulate treatment plan/disposition.
3. Familiarize student with principles of crisis intervention.
Learning Activities: 1. Work in conjunction with a multidisciplinary team to provide evaluation and disposition planning to patients who present to the ED or Psychiatry Walk-In clinic in a psychiatric emergency. Patient may present on a voluntary or involuntary basis.
2. Attend ER conference, weekly staff meeting, community psychiatry lecture, Grand Rounds, daily check-out rounds (8:30 and 4:30).

Conference: 2 hours, Clinic: 15 hours, ER: 15-20 hours. Some evening call. No overnight call.
Evaluation: Dr. Nicholas and Ms McIntyre will consult with other team members, attendings, and residents to make a formal evaluation.
