Evaluation Forms
The evaluation form is to be completed at the end of your orthopaedic rotation. This form must be completed and returned to Karen Gilliam, 2nd Main Hospital, Orthopaedic Clinic within one week after the end of your clerkship or elective rotation.
Remember that the evaluation is anonymous--no signature, please, and you may seal it in an envelope to ensure confidentiality if you wish. These evaluations are not seen by the faculty until your grades have been turned in and thus have no bearing on the evaluation of your performance. Please Note: Your grade may not be released until you have delivered the evaluation to this office.
