There are 4 steps/pages to be filled out. On the second page, you can enter the designation for your gift. Please designate the "School of Medicine" as the "University Designation," and in the next drop-down box, please choose the very last entry called "Other (designate in "Other Instructions" box below)."Please enter "The Center for Aging and Health " in that box.
