Advance Care Planning
Advance Care Planning (ACP) is a process of preparing for the end of life. It enables a person to prepare for a "good death," one that is as comfortable as possible and determined by his or her values and wishes. This educational module provides comprehensive instruction on advance care planning. It is written primarily for health professionals and health profession students and might be most useful to them in their clinical years. Learners should have a knowledgeable and experienced mentor to demonstrate how this process can be applied in clinical situations.
