AOE Webinar – Racial/Ethnic Biases in Clerkship Grading: A Systematic Review and Meta-analysis

After pilot data from the UNC Class of 2019 demonstrated strong racial/ethnic disparities in clerkship grades (White students comprised 60% of the class but 73% of the top quartile), we undertook a meta-analysis of racial/ethnic biases in clerkship grading. This meta-analysis demonstrated strong racial/ethnic biases in clinical evaluations, standardized tests, and overall clerkship grades that did not fully reduce after introducing controlling factors, implying the existence of structural racism – likely mediated by stereotype threat and implicit or explicit biases – in clerkship grading. We contextualize the effects of these significant disparities in clerkship grading by discussing downstream disparities in AOA honors society eligibility and residency match success, both of which threaten the safety and academic progression of our students and, in turn, threaten patients through the deprivation of identity-concordant care in the most competitive residency fields. We will also provide general background on ways peer medical schools have combated these disparities, including Pass/Fail grading and reducing the influence of shelf exams, in anticipation of the formation of our Task Force on Competency Based Grading.
