The 4 documents listed below MUST be on file with the APP Center prior to the start of a student\u2019s clinical rotation in order for the student to be cleared to begin the clinical rotation.<\/p>\n

The 4 documents must be provided to the APP Center via a following Qualtrics link that you will request from the APP Student Placement Manager during on boarding.<\/p>\n

    \n
  1. Student\u2019s Professional Liability COI<\/strong> (if covered under a policy that covers all students, submit that COI.\u00a0 If individual policy, submit that COI)<\/li>\n
  2. LMS Transcript<\/strong> (Student must “Export to PDF” or “Print to PDF”\u00a0 their LMS transcript from their LMS account after completing the HCS Regulatory Compliance Module and EPIC Training Modules.<\/li>\n
  3. Compliance Summary<\/strong> (ONE of the following)\n
      \n
    1. Summary Immunization Report from school\u2019s compliance database or vendor that includes the same information listed on the Compliance Checklist webpage (OR)<\/li>\n
    2. Letter of Good Standing on school letterhead that includes the same information listed on the Compliance Checklist webpage (OR)<\/li>\n
    3. Completed Passport Form (contact Jane.Kerwin@unchealth.unc.edu) to request the Passport Form<\/li>\n<\/ol>\n<\/li>\n
    4. Flu Vaccine Document<\/strong><\/li>\n<\/ol>\n
