This is followed by a 15-minute formal didactic PowerPoint presentation\/Q&amp;A on a scheduled topic presented by the Subject Matter Experts. This is followed by a participant presenting per-submitted cases for discussion with the entire group. You will be asked to volunteer for specific days to present cases.<\/li>\n<li>Help identify and invite other health care systems or practices that make up your medical neighborhood<\/li>\n<li>Identify a primary contact at your site to assist in reviewing and assessing needs<\/li>\n<li>Ensure that your team participates fully in each ECHO session and completes any requested forms\/evaluation\/survey documents as needed.<\/div><\/li>\n<\/ul>\n<span class=\"collapseomatic \" id=\"id6a26002b182b0\"  tabindex=\"0\" title=\"What technology or materials are required to participate?\"    >What technology or materials are required to participate?<\/span><div id=\"target-id6a26002b182b0\" class=\"collapseomatic_content \">\nSessions will be held digitally via Zoom so participants are required to have a Zoom account, access to the internet, and a computer, phone, or other device that can stream video and audio.<\/div>\n<span class=\"collapseomatic \" id=\"id6a26002b18322\"  tabindex=\"0\" title=\"How to install Zoom?\"    >How to install Zoom?<\/span><div id=\"target-id6a26002b18322\" class=\"collapseomatic_content \">\nYou may download the free version of the Zoom client for PC or Mac by visiting <a href=\"https:\/\/zoom.us\/download\">https:\/\/zoom.us\/download<\/a> and selecting Zoom Client for Meetings. We will be collecting data from participants before and after the intervention to measure the effectiveness of the program. <\/div>\n<h1>Patient FAQs<\/h1>\n<span class=\"collapseomatic \" id=\"id6a26002b184a1\"  tabindex=\"0\" title=\"What is expected of participants?\"    >What is expected of participants?<\/span><div id=\"target-id6a26002b184a1\" class=\"collapseomatic_content \">\n<p>You will be asked to complete online questionnaires that will ask about your knowledge, experience and concerns about managing your AFib. Completing this survey should take 30 minutes or less and will be completed two times, one month apart. You will then be offered the opportunity to participate in an online patient education group program for patients with Afib through virtual\/telehealth group sessions. These sessions will be led by a cardiologist (Dr. Gehi) who specializes in the treatment of AFib and a team of multidisciplinary experts. <\/div>\n<span class=\"collapseomatic \" id=\"id6a26002b1850d\"  tabindex=\"0\" title=\"How is a AFib at Home session structured?\"    >How is a AFib at Home session structured?<\/span><div id=\"target-id6a26002b1850d\" class=\"collapseomatic_content \">\n<p>Virtual learning sessions take place via real-time, interactive videoconferencing, using a PC\/Mac, laptop, tablet, or smart phone equipped with a webcam, and a versatile, user-friendly, HIPAA-compliant,cloud-based software application called Zoom.
