To support and empower clinical research personnel in conducting safe, efficient, and high-quality clinical research, the CRSO is developing a series of self-assessment tools. Self-assessments can be utilized to proactively discover strengths and areas for improvement of clinical research activities at any timepoint during a study or in preparation for a monitoring visit or audit.

The self-assessments are provided in a fillable PDF format to allow users to enter information electronically into the form using Adobe Acrobat Reader. Alternatively, print a copy of the form and fill out by hand.

IMPORTANT: For full functionality of the form, use the latest version of Adobe Acrobat Reader and avoid completing the form online within your web browser using an alternate PDF viewer. Before starting the assessment, save the form to a UNC drive without opening the file online within your web browser:

  • On Windows-based PCs: Simply right-click on the hyperlink for the self-assessment form, choose the Save Link As… option, select a UNC folder, set Adobe Acrobat Document as the file type (*.pdf), and then click the Save button.
  • On Macintosh systems: Hold down the Control (Ctrl) key on the keyboard and use the mouse to click on the hyperlink for the self-assessment form, click the Download Linked File As… or the Save Link As… option, select a folder from your Where: options, set Adobe Acrobat Document as the file type (*.pdf), and then click the Save button.

Once you have saved the form, locate the saved form and right-click to open with Adobe Acrobat Reader. Follow the instructions at the top of the form to complete the assessment.
