Item\/items must be purchasable by US Dollars. Please provide your <a href=\"https:\/\/www.med.unc.edu\/cwhr\/pi-portal\/award-managers\/\">Award Manager<\/a>\u00a0with the following information:<\/p>\n<ul>\n<li>Vendor name and address (if vendor is not currently in system a W-9 will be required)<\/li>\n<li>A description and part number of the item you would like ordered<\/li>\n<li>The price of the item<\/li>\n<li>Quantity you would like ordered<\/li>\n<li>Unit of measure that matches the price<\/li>\n<li>Project ID number or Chartfield string to charge<\/li>\n<li>Shipping address<\/li>\n<li>Any quotes from Vendor<\/li>\n<li>Justification of the purchase (ie: what the purchase is for and why this particular vendor was chosen)<\/li>\n<li>Indicate if the order is associated with a State Contract or University Term Contract (UTC.)<\/li>\n<li>Complete the Sole Source\/Waiver of Competition back up documentation is required If only one specific vendor can be used<\/li>\n<\/ul>\n<p>If an order over $5,000 is placed and services or goods are received before a purchase requisition is processed an explanation of why the misuse occurred, supporting documentation, and invoice must be submitted to your <a href=\"https:\/\/www.med.unc.edu\/cwhr\/pi-portal\/award-managers\/\">Award Manager<\/a>.<\/p>\n<p><strong>Link<\/strong>: <a class=\"external-link\" title=\"\" href=\"https:\/\/ccinfo.unc.edu\/files\/2017\/07\/PurchaseReqsJuly2017.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Purchase Requisitions and Receiving Guide<\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Purchase requisitions are required for any order over $5,000, whether or not the purchase is made from a vendor in the vendor catalog.
