These changes are designed to improve the aesthetic appearance of the nose, but may also involve techniques that are designed to improve breathing through the nose.

The goal of rhinoplasty is to alter the shape of the nose as the patient desires, but create a natural nasal appearance, and avoid the “operated” appearance some rhinoplasty surgeries can produce. Of course, function must also be considered in rhinoplasty, and may limit some of the changes that are desired.

What are the biggest advantages and disadvantages of the endonasal technique?

The biggest advantage is the avoidance of raising skin over the nasal tip. This allows for faster healing and less swelling in the tip. It also allows for maintenance of the tip structure and inherent support.

The biggest disadvantage is that it may not allow for more complex alterations in the shape or position of the nasal tip.

How is the endonasal approach performed?

Although most rhinoplasty is performed under general anesthesia, in certain situations it may be performed under local anesthesia. The incisions are carefully placed inside the nose at the junction of the nasal tip and the upper part of the nose. This approach allows for the desired alterations to be performed, as in open rhinoplasty.

What are possible side effects from endonasal rhinoplasty?

Similar to any surgery, bleeding and infection are risks but these are very low (less than 10%). Asymmetry, numbness, adverse scarring, changes in nasal breathing, and dissatisfaction with the appearance of the nose are additional low risks of rhinoplasty in general.

While bruising and visible swelling after surgery, which gradually subsides over a few days to a few weeks, the nose continues to heal over at least a year following surgery. During that time, the skin envelope “shrink wraps” to the underlying cartilage and bony framework, sometimes revealing small irregularities in the contours of the nose.

If you would like to schedule a consultation at the UNC Center for Facial Aesthetics, please call 984-974-2255 to make an appointment.

As with open rhinoplasty, this “closed” technique allows for alterations in the shape of the nose.
