UNC Otolaryngology/Head & Neck Surgery is dedicated to developing and maintaining strong working relationships with referring health care professionals and to making the referral process as efficient as possible.
We also recognize the importance of the relationship that you have with your patients. We seek to honor that relationship by providing the best possible care for your patients and by keeping you informed of your patients’ care.
How to Refer a patient:
Referring a patient with a confirmed/suspicious Head & Neck Cancer
To refer a patient to our clinic with a confirmed head & neck cancer please fax over their health records related to the referral including pathology/laboratory reports, radiology reports, operative reports and first and last clinic notes, demographics and a face sheet with the diagnosis and physician’s name if the referring provider is requesting one. We recognize the importance and urgency of these referrals and our goal is to have these patients scheduled with a provider within 10 days of receiving the referral. Please be aware some insurance companies will require a referral or prior approval before we can schedule an appointment. As we don’t want to delay a patient’s care, our schedulers will hold a time for the patient with the understanding the authorization is being obtained by the referring provider. It’s always a good idea to also follow up with our scheduler to ensure the records were received.

To refer a patient to our clinic please fax over their health records related to the referral, demographics and a face sheet with the diagnosis and physicians name if the referring provider is requesting one. For URGENT referrals, please fax over the appropriate documents and contact the clinic directly to help expedite this process. Please be aware some insurance companies will require a referral or prior approval before we can schedule an appointment. All authorizations should be expedited by the referring provider.
If patients have had any scans or radiology test performed related to the diagnosis please make sure these are provided to the patient prior to their appointment and they are made aware these are pertinent to bring to their appointment.
