All portions must be completed in order to consider your candidacy.<\/p>\n<ol>\n<li>Apply through\u00a0<a href=\"http:\/\/www.aamc.org\/students\/eras\/\">ERAS<\/a>\n<ol>\n<li>Include a 2-page personal statement describing your specific interests in preventive medicine and your goals for this training.<\/li>\n<li>Attach\u00a0a current curriculum vitae, including a list of honors, publications, and summary of prior work.<\/li>\n<li>Through ERAS, please include 3 letters of recommendation, including one from the\u00a0program director of your clinical residency program. The other two should be from medical staff of your residency training institution. \u00a0Please make sure all letters are from your\u00a0<u>most recent<\/u>\u00a0ACGME accredited residency program or fellowship.<\/li>\n<li>Include your state medical license information.<\/li>\n<\/ol>\n<\/li>\n<li>Apply through the<a href=\"https:\/\/www.uncmedicalcenter.org\/uncmc\/professional-education-services\/office-of-graduate-medical-education\/applicants\/\">\u00a0Office of Graduate Medical Education.<\/a>\u00a0 Please be aware that this application is deeply detailed.\u00a0 All &#8220;N\/A&#8221; boxes much be checked, and all questions must be answered prior to final submission in order for your application to be valid.\u00a0 You may send a draft of the form to Amanda Harvey (details below) for editing.<\/li>\n<li>Apply online to the UNC Graduate School at\u00a0<a href=\"http:\/\/gradschool.unc.edu\/\">http:\/\/gradschool.unc.edu<\/a>\u00a0for application to the desired department\/program in the School of Public Health. You must apply separately to the SPH.
