{"id":1218,"date":"2015-10-26T16:28:54","date_gmt":"2015-10-26T20:28:54","guid":{"rendered":"https:\/\/med-fammed.sites.unc.edu\/shaken-baby-prevention-effort-reduces-crying-related-calls-to-nurse-advice-line\/"},"modified":"2017-12-18T14:55:47","modified_gmt":"2017-12-18T19:55:47","slug":"shaken-baby-prevention-effort-reduces-crying-related-calls-to-nurse-advice-line","status":"publish","type":"post","link":"https:\/\/www.med.unc.edu\/fammed\/2015\/10\/26\/shaken-baby-prevention-effort-reduces-crying-related-calls-to-nurse-advice-line\/","title":{"rendered":"Shaken baby prevention effort reduces crying-related calls to nurse advice line"},"content":{"rendered":"<p><!-- description --><\/p>\n<p class=\"lead\">The study found no reduction in state-level rates of abusive head trauma (AHT) or \u201cshaken baby syndrome.\u201d Dr. Adam Zolotor from UNC Family Medicine is lead author of the article published on October 26, 2015 in JAMA Pediatrics.<\/p>\n<div class=\"image-section\">\n<div class=\"newsitem\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright\" src=\"\/fammedfiles\/2017\/11\/shaken-baby-prevention-effort-reduces-crying-related-calls-to-nurse-advice-line-image2.jpeg\" width=\"212\" height=\"297\" \/><\/div>\n<\/div>\n<p>A new evaluation of a statewide shaken baby prevention effort found that the number of calls to a nurse advice line from North Carolina parents who called because of a crying baby were reduced in the first 2 years after the intervention was implemented in 2007.<\/p>\n<p>However, the study, published Oct. 26, 2015 in <a class=\"external-link\" title=\"\" href=\"http:\/\/archpedi.jamanetwork.com\/journal.aspx\" target=\"_blank\" rel=\"noopener noreferrer\"><em>JAMA Pediatrics<\/em><\/a>, did not find a statistically significant reduction in the number of abusive head trauma (AHT) or \u201cshaken baby\u201d cases in North Carolina during the same period.<\/p>\n<p>\u201cWe found this project was a successful implementation of a multi-stage prevention program that reached 88 percent of North Carolina mothers over three and a half years,\u201d said <a class=\"external-link\" title=\"\" href=\"http:\/\/news.unchealthcare.org\/news\/2015\/april\/nciom-names-zolotor-new-president-and-ceo\" target=\"_blank\" rel=\"noopener noreferrer\">Adam Zolotor, MD, DrPH<\/a>, lead author of the study and Associate Professor of Family Medicine in the University of North Carolina School of Medicine.<\/p>\n<p>\u201cIn addition, we found that two years after implementation, the number of calls from parents of crying children to the nurse advice line declined by 20 percent for children younger than 3 months and by 12 percent for children ages 3 months to 12 months,\u201d Zolotor said.<\/p>\n<p>The study found no association between the intervention and state-level rates of abusive head trauma. \u201cThis does not mean that the intervention does not work,\u201d Zolotor said. \u201cIt simply means that our study did not demonstrate that it did work, and additional studies are needed to answer that question definitively.\u201d<\/p>\n<p>\u201cThere are many reasons that we may not have shown a decrease in rates of abusive head trauma, including the recession, the fact that this is a rare problem, and other factors that we did not observe,&#8221; Zolotor said. &#8220;However, the application of an economic modeling technique allowed us to attempt to control for some of these weaknesses. Future research may consider more intensive interventions or interventions targeted to high-risk families.\u201d<\/p>\n<p><a class=\"external-link\" title=\"\" href=\"http:\/\/www.ucdenver.edu\/academics\/colleges\/medicalschool\/departments\/pediatrics\/subs\/can\/faculty\/staffpages\/Pages\/Des-Runyan.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">Desmond Runyan, MD, DrPH<\/a>, director of the Kempe Center for the Prevention and Treatment of child Abuse and Neglect at the University of Colorado, said, \u201cThe intervention took place during the most significant economic recession since the great depression. Four previous studies have shown the recession to be associated with increased rates of abusive head trauma. In North Carolina, we may not have shown a decrease, but there was no increase in rates of abusive head trauma.\u201d Dr. Runyan was chair of Social Medicine and professor of Pediatrics in the UNC School of Medicine <a class=\"external-link\" title=\"\" href=\"http:\/\/news.unchealthcare.org\/news\/2008\/Jan\/shakenbaby\" target=\"_self\">when the study began<\/a> and is a co-author of the article in <em>JAMA Pediatrics<\/em>.<\/p>\n<p>Parents of babies in the study were provided with an intervention program called the \u201cPeriod of PURPLE Crying,\u201d which was developed by Dr. Ronald Barr, a professor of community child health research and a developmental pediatrician at the University of British Columbia, and Marilyn Barr, founder and executive director of the National Center on Shaken Baby Syndrome. Both Dr. Barr and Marilyn Barr collaborated with the North Carolina project.<\/p>\n<p>The program includes hospital and health care provider-based parent education. The program educates parents and caregivers about the hazards of shaking and gives them alternatives to use when they feel they need a respite from a crying baby, such as handing off the baby to another caregiver.<\/p>\n<p>In addition to Zolotor and Runyan, <a class=\"external-link\" title=\"\" href=\"http:\/\/mj.unc.edu\/directory\/faculty\/heidi-hennink-kaminski\" target=\"_blank\" rel=\"noopener noreferrer\">Heidi Hennink-Kaminski, PhD<\/a>, of the UNC School of Media and Journalism and <a class=\"external-link\" title=\"\" href=\"http:\/\/www.uq.edu.au\/sjc\/libby-mitchell\" target=\"_blank\" rel=\"noopener noreferrer\">Elizabeth Mitchell, PhD<\/a> of the University of Queensland (formerly at UNC) worked with the National Center on Shaken Baby Syndrome to develop a statewide media campaign to address social norms about shaking and reinforce program messages through caregivers, family, and friends. The Center for Child and Family Health \u2013 a collaborative effort involving Duke University Medical Center, North Carolina Central University and the UNC School of Medicine led the implementation of the project. Dr. Robert Murphy, the center\u2019s executive director and an Associate Professor of Psychiatry at Duke and Dr. Kelly Sullivan, the center\u2019s director of mental health services and Period of PURPLE Crying program manager, led a statewide leadership team that fostered the effort in the 86 hospitals where children are born, as well as in community settings across the state.<\/p>\n<p>Funding for the project came from three sources. The Centers for Disease Control and Prevention provided a five-year, $2.9 million grant and the Doris Duke Charitable Foundation provided a $2 million grant, both to the UNC Injury Prevention Research Center. A third grant of $2 million from The Duke Endowment went to the Center for Child and Family Health, with Dr. Murphy as principal investigator, to fund implementation.
