The model is grounded in three fundamental principles:<\/p>\n<ol>\n<li>to begin all planning with the end in mind<\/li>\n<li>to promote active client participation and support psychological well-being<\/li>\n<li>to synchronize treatment components into a holistic plan<span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559740&quot;:259}\">\u00a0<\/span>Our vision is to develop applications across the neurorehabilitation continuum<\/li>\n<\/ol>\n<h3>Collaborative FOURC<\/h3>\n<p>The first version of the FOURC model was developed in collaboration with clinicians working in outpatient settings. It was published as a 2019 tutorial in the American Journal of Speech Language Pathology. This model was designed to result in meaningful communication outcomes for clients and to support active planning and accountability for those who have lived with their communication disorder for a while. The anticipated outcome is to ensure that patients are informed about their communication disorder, able to participate as decision-makers in their own care and discharge plan, and have had positive experiences communicating even during the earliest stages of recovery. We are busy working on a tutorial that will\u00a0 hopefully come out in 2026. In the meantime, we are offering print resources on the <a href=\"https:\/\/www.med.unc.edu\/healthsciences\/sphs\/card\/resources\/aphasia-goals\/acutecarefourc\/\">Acute Care FOURC page<\/a>. Please check continuing education offerings, including at the ASHA 2026 convention.<\/p>\n<\/div>\n<h2>The Aphasia Goal Pool<\/h2>\n<p>We started the Aphasia Goal Pool in the spring of 2015 as a way to learn from the professional community about strategic goal writing for aphasia. Over the first 34 months, we asked speech-language pathologists to send us examples of goals they were using in their practice. We welcomed any examples as long as they were representative of goals you had actually used in your practice. We collected 1,075 goals in this first round. By following the <a href=\"https:\/\/www.med.unc.edu\/healthsciences\/sphs\/card\/resources\/aphasia-goals\/goal-pool\/\">Aphasia Goal Pool tab<\/a><strong>, <\/strong>you can learn about some of the trends and strategies we noticed.
